Nasogastric tubes are typically inserted while the patient is awake and aware of the procedure. Patients are encouraged to remain calm, relaxed, and to breathe normally throughout the process.

Patients are positioned upright or with their head elevated during the procedure. The tube length is selected based on individual body measurements and age. The tip of the tube is lubricated to facilitate smooth passage. Patients are instructed to look downward with their chin tucked toward their chest. The tube is then carefully guided through the nostril and nasal passage. Swallowing or sipping water may assist in advancing the tube to the correct position. Placement is confirmed using an X-ray or by testing stomach contents.

In some cases, fluoroscopy guidance is used to accurately place the tube as it is advanced through the nose, esophagus, and stomach. A guidewire may also be utilized to assist with correct placement.
