Oesophageal manometry is a diagnostic test used to evaluate the efficiency of the oesophagus in transporting food to the stomach. Impairments in the motor function of the oesophagus are classified as oesophageal motility disorders. The oesophagus, a muscular tube connecting the mouth to the stomach, relies on coordinated muscle contractions to move food downward and prevent its return. Disruptions in this process due to motility disorders can lead to symptoms such as dysphagia and gastroesophageal reflux.
Oesophageal manometry is indicated for the assessment of oesophageal motility, particularly in individuals experiencing symptoms such as dysphagia or difficulty retaining food in the stomach. Typically, clinicians first request imaging studies to identify structural abnormalities. Motility testing is pursued if imaging results do not provide a definitive diagnosis.
Indications for oesophageal motility testing may include the following symptoms:
- Pain when swallowing
- Regurgitation of food
- Persistent acid reflux that fails to respond to medical management
- Heartburn
During the procedure, the patient remains seated in an upright position while a nurse applies a topical anaesthetic to the nostrils and oropharynx. A thin catheter is inserted through the nostril into the oesophagus as the patient swallows, and is advanced to the stomach. At designated intervals, the patient is instructed to take sips of water or saline solution to facilitate measurement of oesophageal pressure and pH levels. Pressure sensors on the catheter record the contractile and relaxation activity of the oesophageal muscles during swallowing. The resulting data provide detailed information on oesophageal function, enabling clinicians to recommend targeted treatments.
